Position: Community Health Worker…
Location: Remote/WFH – Must be Resident of North Carolina
Duration: 3 Month Contract to start, potential for extension and/or conversion to FTE
Rate: $22.00/Hr
Start Date: 11/04/2024
Important: Must be local resident of North Carolina
Primary Responsibilities:
As a Community Health Worker (CHW), you will act in a liaison role with Medicaid members to ensure appropriate services are obtained via the Healthy Opportunities Pilot Program in addition to care coordination and member education.
Primary Responsibilities:
? Perform social needs assessment for members
? Utilize the NCCARE360 platform for documentation and referral purposes
? Clearly and effectively communicate with members and Human Service Organizations to arrange services
? Linking members to applicable community resources
? Telephonic outreach to targeted members
? Proactively engage the member to manage their care
? Provide member education
? Support transitions of care
? Create a positive and engaging experience for the member
? Help to keep members compliant with their care plans
? Partner with and support care team by completing delegated tasks
? Knowledge and continued learning of community cultures and values
? Conduct individual-level screenings to capture member’s current health care needs (e.g. Transitions of Care, Health Risk assessments, Pilot Eligibility Service Assessment (PESA)
? Engage member to participate in the screening process and collaboratively develop plan of care based on their individual needs, preferences, and objectives
Required Qualifications:
? High School Diploma / GED (or higher)
? Must be a North Carolina resident
? Must have a designated workspace inside the home with the ability to access high-speed internet
? Must be proficient in Outlook and Microsoft Office
? Must possess excellent organizational skills
? Must be proficient in clear, customer service focused communication
Would be a Plus to Have:
? Bachelor?s Degree (or higher) in Social Work and / or Health Care Administration
? Licensed Practical Nurse (LPN), Certified Nursing Assistant / Home Health Aide, Medical Assistant
? CHW Accreditation
? Experience working in Managed Care and/or Social Needs assessment
? Knowledge of Medicaid population
Hours:
Remote Work/telecommute and must live in North Carolina (EST)
Hours: 8 AM ? 5 PM EST, Monday-Friday, with a 1 hour lunch
